Free Living Wills and Advance Health Care Directives

A living will, sometimes referred to as an “Advance Health Care Directive”, provides a directive to physicians and health care providers regarding your wishes as to specific medical treatments or procedures to be used in your treatment in the event you do not have the mental or physical capacity to do so on your own behalf.  After completing the form in several duplicate originals, you can provide an original document to your family and your physician.  The Living Will does not replace or affect your regular will and it is wise to consult your attorney.  The AUIC does not engage in the practice of law and only provides these forms for education and convenience.

You can choose your state of residence for the Living Will or Advance Medical Directive Form.  These forms are free and have been compiled from other websites provided by state bar associations or other associations interested in promoting the benefits of advance planning using a Living Will or an Advanced Medical Directive.  These documents can be edited on your computer and printed for execution or can be saved to your computer by choosing “Save Target as” and later filling in and editing the forms using your word processing software.
